• This online appointment scheduling system is for existing patients only.

• Prospective new patients: Please e-mail frontdesk@martinjensenmd.com to request an appointment, and we will get back to you. The initial consultation will be in-person. For follow-up appointments, there will be a telemedicine option.

• Existing patients on controlled substances (such as Adderall, Xanax, Ambien, etc.) are required to have at least one annual in-person appointment.

• Patients who have been sick, or exposed to someone sick, within two weeks of an in-person appointment should change their appointment to telemedicine or reschedule the in-person appointment for a later date.

• A cancellation fee will be charged for no-shows and appointments canceled within 24 hours of a scheduled appointment.
