Please complete and return all forms prior to your first appointment. Forms can be emailed, mailed, or dropped in our mail slot. 

If possible, please prepare the following prior to your first appointment:

• A list of past and current psychiatric medications: dosage, duration, and outcome

• A list of your medical conditions, current medications, and nutritional supplements

• Laboratory tests from the past two years, or beyond, if relevant

• Brain MRI, SPECT study or EEG report, if this has been done

• Psychological test reports if they exist
